PATIENT INFORMATION
Thank you for choosing That Broadway Smile for your pediatric dental needs. During their first visit, our office will provide your child with a comprehensive oral evaluation. If your child(ren) were previously patients at another dental practice, or if you have moved into the area, please request a copy of their records be sent to our office. These records can be emailed to us at info@thatbroadwaysmile.com or faxed to us at 516-753-9027.
At the initial visit, please make sure you bring along your insurance card and fill out any paperwork online prior to your appointment by clicking the Online Patient Registration Form below.

First Visit
According to the American Academy of Pediatric Dentistry, every child should visit a Pediatric Dentist at approximately 12 months of age or when the 1st tooth erupts. After the initial visit, visiting the dentist every 6 months will help your child maintain optimal dental health. Click below to learn more about what will happen at your child’s first visit!

To save time during your first visit, we ask that parents complete their child’s paperwork online via the below link. We also ask that existing patients update their paperwork if the insurance provider has changed since their last visit.
After Care
After your child receives treatment in our office, our doctors and staff members will discuss the after care instructions with you in full detail. In addition, we have also put these instructions on our website in-case you forget. If there is anything you may still have questions about, please call us and we will be happy to help.
